This article explains what the Contact Form is, and the one piece of setup it needs from you — the form has to be embedded on your website, in place of the contact form you're using now.
What happens once someone submits it, and what your team does with the inquiry, are covered in separate articles.
What the Contact Form is
The Contact Form is the form on your website that someone fills in when they want to get in touch, rebuilt so that ClinicMonk™ can act on it.
When it's submitted, the form is replaced in place by a short confirmation, and several things happen at once behind it. The person gets a text and an email within moments, both offering your free 20-minute consult and inviting them to reply. A lead is created in Client Journey at New Lead, with its source recorded as Contact Form. Your clinic administrator gets an email telling them there's an inquiry waiting. And if the person arrived from a Google ad, that's recorded on the lead, which is what feeds the Google Ads figures on your Digital Marketing & Referrals dashboard.
None of that is possible with an ordinary website contact form. An ordinary form sends an email to a mailbox and stops there. This one starts the conversation on its own, in the minutes after someone reaches out, which is usually when they're deciding whether to reach out to somebody else as well.
The one thing it needs from you
The form only does any of this if it's the form on your website. ClinicMonk™ gives you a short piece of embed code; your website has to carry it.
Until that's done, inquiries keep arriving through your old form, into a mailbox, with no lead, no automatic text or email, and nothing on your dashboards. This is a one-time job, and it's usually the last step of setup.
Replacing the form you already have
The ClinicMonk™ form replaces your existing contact form. It doesn't sit alongside it.
Two forms on one website means inquiries arrive through both, and the ones that come through the old form are invisible to ClinicMonk™ — no automatic reply, no lead, and a gap in the numbers you're using to judge your marketing. Your show rates and conversion rates are only as accurate as the inquiries the system can see.
So it has to be replaced everywhere it appears. Most clinic websites have a contact form in more than one place: the contact page, the footer, individual service or therapist pages, sometimes a pop-up. Every one of them needs the new form.
Getting your embed code
You only need to do this once, and you don't need to understand the code itself. You're copying it so you can pass it on.
Sign in to ClinicMonk™.
Click Funnels and Forms in the left sidebar.
Click Forms.
Select the Contact Form.
Click Integrate, at the top right.
Embed Code should already be selected. If it isn't, select it.
Click Copy embed code.
The code is now on your clipboard. Paste it somewhere you can get it back — an email to yourself, or straight into the message you're about to send your web manager — because clipboards don't hold onto things for long.
Handing it to your web manager
Most clinic owners don't do this part themselves. If someone else looks after your website — a web manager, a designer, or whoever built the site — send them the embed code and ask them to use it to replace the existing contact form, in every place a contact form appears on the site.
That's everything they need. Embed code is a standard thing to be given, it works in every common website builder, and it needs nothing from ClinicMonk™ at their end: no plugin, no account, no login.
One thing worth saying to them explicitly: replace, don't add. It's a reasonable assumption on their part that a new form is meant to go somewhere new, and it's the mistake that's hardest to notice afterwards, because everything looks like it's working.
Check it works
Do this once, after the new form is live. A test submission is a real submission — it creates a real lead and sends real messages — so use your own name, email address and mobile number, and the messages come to you.
Go to your website's contact page and submit the form with your own details.
Check the form is replaced in place by the confirmation message, rather than the page you had before.
Check you receive the text, and then the email with the subject Thanks for reaching out.
Check your clinic administrator receives the email with the subject Action Required: New Contact Form Submitted, and that the link inside it opens the submission.
Open Client Journey and confirm the lead is there at New Lead, with its source recorded as Contact Form.
Visit every other page that has a contact form on it and confirm each one is the new form.
That last step is the one to be thorough about. It's common for the contact page to be updated and the footer form to be left behind.
When you're done, you can set the test lead's status to Lost so it isn't sitting on your board.
What happens next
With the form live, the rest runs on its own. Two other articles in this section cover it: everything ClinicMonk™ does the moment a form is submitted, and what you or your team need to do so the inquiry gets a real reply.
