First, connect your contact form to your website
Before you start, make sure your ClinicMonk™ contact form is connected to your website. If it isn’t connected yet, send this website setup guide to your website team and ask them to embed your ClinicMonk™ contact form.
Once it’s connected, you can update the form here in ClinicMonk™.
Before you make changes
Keep the name, email, phone and text-messaging consent questions. Your follow-up setup may rely on them. If you’re unsure about a change, use the support options at the bottom of this article.
Open your contact form
Click Funnels and Forms in the left menu.
Choose Forms.
Click Contact Form to open it.
Click the screenshot to see it full size.
Add or edit a question
4. To add a question, drag the question type from the left panel onto your form. For example, choose Checkbox when people can select more than one answer. If you’re editing an existing question, skip this step.
5. Click the question on your form, then click its gear icon to open the settings.
6. Enter your question under Label. For questions with choices, you’ll find this on the Content tab.
Click the screenshot to see it full size.
7. If your question has a list of answers, open Options to enter or update the choices.
Changing an existing question’s choices can affect other forms that use that same question. Ask support first if you’re unsure.
8. Review your changes, then click Save.
9. Click Preview to check how the form looks.
Click the screenshot to see it full size.
How do I let people select the services they’re interested in?
You can add a list of services instead of asking visitors to type their answer into an open-ended question.
Open your Contact Form using the steps above.
Add a Checkbox question if visitors should be able to choose more than 1 service.
Set the label to Please select the service(s) you are interested in:
Under Options, add each service as a separate choice. For example: individual therapy, couples therapy and assessments.
Keep your existing open-ended question while you check that the new choices work. If you want to replace it, ask support to confirm that removing the old question will not affect your follow-up setup.
Save and preview the form. Select more than 1 service to confirm the question works as intended.
Changing a question’s wording does not change its type. If your current question is a text box, add a Checkbox question to collect multiple selections.
For a new question, ask support to confirm where the answers appear in the contact record and whether any follow-up needs to use them.
Remove a question
Click the question, then the trash icon at its top right. Click Save when you’re ready. Ask support before removing a question that your follow-up process may use.
Good to know
If your website uses this embedded ClinicMonk™ form, saved changes update that form on your website. You don’t need your website team to replace the embed code for each wording change. Check the form on your website after saving.
For new questions, ask support to confirm where the answers will appear in the contact record.
I can’t find my contact form. What should I do?
Go to Funnels and Forms → Forms and look for Contact Form. If it is missing, check that you are in the correct clinic account and ask support to locate it. Avoid creating a replacement form before support checks your existing setup.
Will my changes appear on my website?
If your website uses the same embedded ClinicMonk™ form, saved changes appear there too. Open your actual website after saving and check the wording and choices. If it still shows a different form, ask your website person to confirm which form is embedded.
How do I know my form is ready to launch?
For your initial launch, wait until after your second onboarding call and until support confirms your setup is ready. Text messages cannot send until texting verification is approved.
Use your own details for a test submission on your website. Confirm the submission reaches your clinic account, your team receives the expected notification, and the expected follow-up arrives. Check that your new service selections appear in the contact record. A test submission can trigger real messages and usage charges.
If something is missing, ask support to check the form and follow-up before you launch. Previewing the form alone does not confirm the full process works.
